Only submit information and documents you are authorized to share. Check the legal name, EIN, mailing address, representative email, and attachment readability before certifying the form. Keep the application reference after successful submission.

The review screen is not a submission receipt. A confirmation records receipt for review, not approval or a commitment to pay by a particular date. Contact ACF if you need to correct a submitted record.

Linked IRS guidance and public-return PDFs are provided for reference. Third-party services have their own terms; use official contact channels to confirm a request involving your organization.

Historical returns describe their respective tax years. If a link is broken or page information appears inaccurate, report the page and relevant document through Contact Us.
